Lebanon County Correctional Facility candidates are to submit a request to the chaplain asking to be considered for the Jubilee Transition House.
[Candidates from other facilities can make a request by mail or through a chaplain, counselor or parole officer.]
An initial interview will be set up to explain the details of the transition house and determine your eligibility.
If eligible, an intake form will be completed.
Input will be given by the prison treatment team. [Candidates from other facilities will need at least one recommendation from appropriate staff.]
Jubilee Aftercare staff will approve or disapprove.
Upon approval an agreement will be entered into with yourself, parole and Jubilee Ministries.
Upon approval preparations will be made to obtain necessary ID and establish realistic goals.
