To apply for Jubilee's Aftercare Program, please complete the following four (4) requirements.  Click on the appropriate button to access each of these forms.

1.     AFTERCARE INTERVIEW QUESTIONS
2.     PERSONAL LIFE STORY
3.     CHAPLAIN or COUNSELOR RECOMMENDATION
4.     DAILY QUIET TIME JOURNAL (Do 14 days)

Applications will be reviewed by our aftercare committee and we will contact you after the review.
